Services for adults include primary medical care, diagnostic and screening labs, diagnostic X-ray procedures, diagnostic tests, family planning, women's health, immunizations, mental health screenings and treatment, nutritional services, STD testing and treatment, pregnancy testing, case management, dental cleanings and treatment, women's health, work physicals, urgent care and vision care. Patient fees are calculated on a sliding scale that is based on family size and income.

 

Overview of Behavioral Health Services

Behavioral Health offers brief treatment and intervention for adolescents and adults to help improve their quality of life. Services can be offered to address the following:

  • Depression (including elevations on the PHQ-9)
  • Anxiety
  • Healthy living practices
  • Coping with a chronic health condition
  • Managing a chronic health condition. Examples include:
    • Diabetes
    • Hypertension
    • Obesity

If you have a patient in crisis, who needs assistance with improving physical health, or who wishes to make a behavioral change, please contact a Behavioral Health Provider.
